Change of Personal Details

If you change your name, address or telephone number please come to reception and complete a change of details form. We kindly ask patients to keep telephone contact details up to date, especially mobile phone numbers. 


Access To Data

Copies of your medication or any other information needed prior to a hospital appointment may be obtained from reception. If you require full copies of your patient notes, requests must be made my writing and a fee will be charged for copies. Data may be shared with 3rd parties, in accordance with NHS guidelines.


Disabled Facilities

Minfor Surgery is a purposed built practice and provide easy access for the disabled. Parking space, toilet facilities and all doctors and nurse consulting rooms are on the ground floor. 

We work alongside WITTS to book communication support for deaf or foreign speaking patients in requested/required. 


'Did Not Attend' Policy

Due to increases in the number of wasted appointments, through patients failing to attend without informing the surgery, it has become necessary to implement the following policy:

If you fail to attend 3 appointments without prior notice, you may be removed from the practice and will have to find an alternative practice. 

If you cannot attend your appointment for any reason, please let us know as soon as possible. This allows us to offer the appointment to someone else.


Medical Records / Data Protection 

Your medical records are held in the strictest confidence. Information is not passed without your consent unless it is within the confines of the NHS, by legal framework, or is in the public interest. Certain anonymised patient data maybe shared for the purpose of public health and audit, research, teaching and training. This practice is required under the Data Protection Act. 

It is a practice and legal requirement that ALL STAFF maintain confidentiality of patient's records.  


Freedom of Information

The Freedom of Information Act 2000 obliges the practice to produce a publication scheme. A publication scheme is a guide to the 'classes' of information the practice intends to routinely make available.

Complaints

If you have any complaints or suggestions about the running of the practice please address them in writing to Sean Langridge, Practice manager. This can be made via post or email - sean.langridge@wales.nhs.uk. It is also possible for you to telephone the surgery and arrange a meeting. 


Violence and Aggression Policy

Our practice staff are here to help you. Our aim is to be as polite and helpful as possible to all patients.
If you consider that you have been treated unfairly or inappropriately, please ask the reception staff to contact the practice manager, who will be happy to address your concerns. 

However, shouting and swearing at practice staff will not be tolerated under any circumstances and patients who are abusive may be removed from the patient list.
